During the natural path of conception, the couples not only need a properly built and functioning male and female genital organs, healthy eggs and sperm, but also the correct functioning of a number of factors to allow embryo to be properly nourished and implanted on the uterine walls, inside the uterine cavity.

However, if the couples are unable to conceive, even after a year or more of regular unprotected intercourse, they are said to be facing infertility. In such cases, Assisted Reproductive Techniques (ART) like IUI (Intrauterine Insemination), IVF (In-Vitro Fertilization) or ICSI (Intracytoplasmic Sperm Injection) have successfully helped such couples in fulfilling their dream of parenthood.

Path of Natural Conception

About two weeks after a woman has her periods, she ovulates, and her ovaries release one mature egg. The egg can be fertilized within 12 – 24 hours of its release from the ovaries as it travels down the fallopian tube towards the uterus. Once the egg meets the sperm, it combines to form an embryo, and the process is called fertilization or conception. Within 3-4 days of fertilization, the embryo implants itself to the uterine walls and further grows into the placenta and fetus.

Important Directions to the Path of Natural Conception includes:

  • Fertility of Men and Women: One of the important directions to the path of natural conception is that, both men and women should be fertile. Moreover, the important determinants of fertility in women is her regularity in menstrual cycle and ovulation, and that in men is his sperm’s quality in terms of count (number of sperms in semen), motility (ability to move) and morphology (shape and size). For natural conception both men and women trying to conceive should be fertile.

  • Correct Intercourse Timing: The Fertile couples, while trying to conceive should understand the cruciality of intercourse timing. The best time to conceive are the days when the woman ovulates (when the egg is released from the ovaries after the follicles mature), which is also known as her fertile window as she is most fertile during those days. Intercourse during the time women ovulates, increases her conception chances. Moreover, it is best to keep trying every other day, from 5 days after her menstruation till 5 days prior to it.

  • Fertilization: It is the process in which the egg and sperm meet to form embryo. Once the egg is released from the ovary, it travels through the fallopian tubes where it meets the sperm. The sperm penetrates inside the egg and form a cell called embryo. A successful fertilization is a crucial step to help couples conceive.

  • Implantation: Within 3 days of fertilization, the embryo multiplies itself to form a blastocyst, and attaches itself to the uterine walls. This successful attachment of embryo to the uterine walls is called Implantation of Embryo, or in other words, conception of pregnancy.

You are officially pregnant, when the embryo finally attaches itself to the uterine walls and develops into the placenta and fetus.

Path of Conception via IUI/IVF/ICSI

Unlike the path of natural conception, the path of conception via ART procedures like IUI/IVF/ICSI are for those couples who have been facing difficulty conceiving for a year or more, in spite of regular unprotected intercourse.

  • Conception via IUI: Intrauterine Inseminationalso known as IUI is comparatively a non-invasive and less-expensive procedure in which the washed sperm is injected inside the woman’s uterus to implant.

This process is usually carried out in cases of men facing infertility due to low sperm motility, morphology or count, unexplained infertility, sperm’s inability to penetrate through the women’s cervix or cervical mucus, mild endometriosis in women, men facing erectile dysfunction or ejaculation issues etc.

Sometimes, this procedure is carried out without any medications. The doctor in such cases, injects the sperm inside the woman’s uterus during the time of her ovulation. However, it is also sometimes combined with stimulation medications such as HCG (Human Chorionic Gonadotropin) and FSH (Follicle Stimulating Hormones) to prompt the ovaries to release one or more eggs. Ovulation with more than one egg increases the chances of conception.

  • Conception via IVF: In-Vitro Fertilization (IVF) is the procedure in which the fertilization takes place outside the body to assist in the conception of pregnancy. It is the process in which the eggs are retrieved from the ovary and fertilized with sperm in an IVF lab. The embryo thus formed is transferred back to the uterus to implant.

IVF is considered to be the most effective form of assisted reproductive technology which can also be done using donor egg, donor sperm, or both, considering female or male infertility. Moreover, it is known to be an absolute treatment in cases of male and female infertility that includes fallopian tube blockage, ovulation disorders, endometriosis, unexplained infertility, oligospermia, azoospermia, genetic disorders etc.

  • The procedure is basically carried in few important steps.

    • The first step is Retrieval of Eggs by inducing ovulation through stimulation medications.
    • After the procedure of egg retrieval, the Eggs are Fertilizedwith Sperm in an IVF lab.
    • The next step after the formation of embryo is the culturing of those embryos for 3-5 days known as Blastocyst Culture.
    • After the culturing of embryos, they are transferred back to the uterus to implant called Implantation of Embryo.
  • Conception via IUI: The Intracytoplasmic Sperm Injection also known as ICSI treatment is an advanced form of IVF treatment. It is the procedure in which the sperm is injected inside the cytoplasm of a mature egg to increase the chances of conception. This procedure is basically carried out to enhance the conception chances in cases of male factor infertility.

  • The ICSI treatment is usually a procedure to treat severe male factor infertility like men suffering from low sperm count (oligospermia), low sperm motility, or abnormality in sperm morphology etc. or at times when frozen eggs are used for fertilization. In such case one best sperm is chosen and injected inside the cytoplasm of the egg to increase the success chances.

  • The procedure of ICSI treatment is similar to IVF treatment from the Egg Retrieval Procedure to Implantation of Embryo, just the Fertilization procedure is carried out differently by injecting the sperm into eggs.

  • ICSI treatment can be done either as an extension of the existing IVF procedure, or as an independent procedure altogether.

FAQs:

Due to the reduced number of eggs, doctors usually prefer ICSI over IVF as it has greater success rate of fertilization. However, it is unknown whether IVF is more successful over ICSI in achieving pregnancy or not.

IUI can be considered a better option for couples facing mild infertility cases like ovulation disorder, while IVF is a procedure, that is meant to treat both mild and severe male and female factor infertility.

According to several researches and studies, the birth defects of IVF and ICSI is similar to that of naturally born babies, which imply that IVF or ICSI babies are also normal babies.

Although not always, but some studies suggest that women with advanced maternal age undergoing ICSI treatment are at an increased risk of miscarriage.
